Student Accident Insurance Policy Guide
Yonsei University subscribes Student Insurance to prepare students for various safety accidents that may occur during on-campus and off-campus activities and promote student welfare. Please refer to the Student Insurance application manual below for further details.

Student Insurance Information
1. Student Accident Compensation Required
a. Insurance application form and consent for personal information processing
Before November 30, 2015: Form 1
After December 1, 2015: Form 2
b. Accident report (form 3)
c. Receipt of medical expenses
(Hospital-issued receipt, card slip not accepted) medication receipt, diagnostic/care chart receipt
d. Bankbook copy : parents' bankbook copy and resident registration copy attached if the student is a minor under 19 years old
e. Copy of student ID: For foreign students, a copy of the alien registration card is attached.
f. Diagnostic document
Before 30 November, 2015: If the cost of treatment exceeds 1,000,000 won
After 1 December, 2015 : Additional treatment is added If treatment cost is over 100,000 won or if the cost exceeds 1 million won.
g. In case the hospital used the superior hospital room because the standard hospital room was not found: Attach the confirmation certificate of use of the superior hospital (maximum of 7 days)
h. Transaction statement (not accepted card receipt) and attached communication statement when using medical aid
i. For dental prostheses: first medical record and a doctor’s note on prostheses

B. Undergraduates and graduate students should send their applications, consent forms, and all submission documents through visit submission or email to ysas@yonsei.ac.kr.
